What are the fees I have to pay?
There is a $5 fee for main-in record requests for driving or vehicle records. There is a $20 per year fee for main-in record request for copies of microfilm documents.
What information do I need to request a record?

All record requests must include:

  • Record number or title
  • Number of copies you want
  • Business name (if applicable)
  • Telephone number
  • Contact name
  • Street address (records cannot be delivered to a PO box)
Can I request records in bulk?
If you need more than 15 records at once, you need to contact DMV Materials Management by email (DMVASDPublicForms@dmv.ca.gov), fax (916-928-6817), or mail (DMV Materials Management Section, 4201 Sierra Point Dr. Ste. 112, Sacramento, CA 95834-7900).
